November 7, 1814. PCGS states that this wide format note has been severed and repaired. The Newman cataloger writes "This note seems likely to have been issued from the town of Milton in Ulster County, New York." Presumed to be unique since there does not appear to be any other record of another. This first sold for $470 in May 2016 but that seems quite reasonable in light of its rarity, time period and location.
